To enter the draw, simply complete these 2 easy steps:

2. Complete the Referral Form below.

We will contact the physicians that you refer, so be sure to let them know you've provided their names. You may refer as many colleagues as you like - and you will receive an entry in the draw for each one.

Draw Dates & Prize Details
All entries each month are entered into a draw for great prizes like these:

• iPad 2 (32GB) (value: $619)
• Canon EOS Rebel T3 Camera with 18-55mm IS lens (value: $649)
• LG 42" 1080p LCD HDTV (value: $639)
• Breville Die-Cast Programmable Espresso Machine (BES830XL) (value: $449)

Monthly draws will be held on August 31, September 30, October 31, November 30, and December 30. In addition, all referrals will be eligible for our grand prize draw, held in January.

The grand prize is a 5 day / 4 night 'Best of BC' Escape for 2, featuring round-trip transportation to Vancouver, a Rocky Mountaineer Dome Train trip to Whistler, floatplane from Whistler to Victoria, whale watching and garden tours in Victoria, and a BC Ferries trip back to Vancouver. Terms and Conditions
This program is valid for new referrals only, beginning on August 1, 2011. Entries are valid for the monthly draw in the month in which they were submitted, and for the grand prize draw in January. Prizes may not be substituted for cash. The referral program may be subject to changes or modification without notice. Prizes may change due to availability. Winners will be contacted by telephone within 2 weeks of the prize draw date. Winners consent to their names and photos being posted on www.wolfmedical.com.
